its called elephantiasis
a disease caused by small/somewhat microscopic parasites/worms thats blocking our lymphatic system causing the lymphs to expand thereby giving that kind of grotesque appearance
btw, its incurable
this usually occurs in humid/tropical climate countries
the carrier of this disease are mosquitoes
